Identity review
Saved requests remain pending identity review. Submission does not verify your identity or confirm that a data change is complete.
Retention and deletion
Retention depends on the information and service involved. Submitting a deletion request does not automatically erase stored records or provider copies.
Data copies
The form accepts requests for access or a copy of your data. It does not generate an automatic data export.
